Appointment Policies

What to Wear

Wear comfortable, loose-fitting clothing to your session. Shorts and a loose t-shirt are ideal.

Hygiene

Please come bathed, as safety and hygiene are important. Please refrain, too, from wearing any lotions, perfumes or scents of any kind. If work is to be done on the face, please refrain from wearing makeup or lotion on your face and neck.

Cancellation

Your appointment time is scheduled especially for you! To cancel or reschedule your appointment, please call at least 24 hours in advance of your scheduled appointment time. For all appointments missed or canceled with less than a 24 hour notice, full fees will be charged. At this time cash and checks are accepted.

Late Arrival Policy

If you must arrive late, your appointment time will be necessarily shortened by that amount of time and you will be charged the same for a full appointment. Being on time assures you receive the full benefits of our work together.

Payment Policy

All clients are expected to pay in full for services rendered at the time of service either with cash or check.  While I do not accept insurance, flexible or health savings plans typically allow for soft tissue therapies and clients often file for full or partial reimbursement.  I will be happy to provide a receipt via email and you can file with your insurance company at your convenience.

Informed Consent

New clients will be asked to sign an Informed Consent form before treatment begins. Any questions you have should be discussed at this time. Please feel free to request any and all information you need about the services in order to ensure your experience is positive and productive.

Service Disclaimer

I am a licensed massage therapist, and will only perform techniques that assist your body to use its own ability to heal.

A consultation is for educational purposes only. I may make suggestions to you and relay information however that does not mean I am diagnosing or prescribing to you.

You will be asked to sign a standard Informed Consent Agreement as well as Payment Policy Agreement on the initial visit.

Confidentiality and Privacy Policy

Confidentiality Agreement

As a licensed and professional massage therapist I maintain the confidentiality of all client information, unless law or court order mandates disclosure. If necessary, I may discuss your case with your doctor, however, that will only be done with your written permission.

Website Privacy Policy

Protecting your privacy is important. I collect no personal information about you unless you choose to provide that information to me. When you do provide personal information, such as e-mail address, or billing/shipping information, I will keep that information confidential.

The Cranioworks website does not identify visitors. Anonymous tracking information is collected to analyze site and page visitation. The only time the website acquires information from you is when you use the contact form or sign-up for the newsletter.

Cranioworks will not disclose, give, sell or transfer any personal information about our visitors or clients unless it is required by law.
